You can request an appointment online or call our office. We'll gather your insurance information and send paperwork before your first visit. Most new patients are seen within 1–2 weeks.
We accept most major plans including Medicare, Medicaid, BlueCross BlueShield, Aetna, Anthem, UnitedHealthcare, and Humana. Contact our office with questions about your specific plan.
Bring a valid photo ID, insurance card(s), a list of current medications, any relevant medical records or referrals, and completed new patient forms. Please arrive 10 minutes early.
Yes! We offer secure video telehealth for many appointment types including follow-ups, mental health sessions, and minor illness consultations. Ask about telehealth when scheduling.
Refills can be requested through the patient portal, by calling our office, or via pharmacy electronic request. Allow 48–72 hours for processing. Controlled substances require an in-person visit.
